Beetle Larva + ?

Okay, as if a poo-brella isn't icky enough, this larva also appears to have some sort of tiny worm thing on it. I didn't notice it when I was taking the photos, but in looking at the photos I can see that the thing changes position. It's not part of the larva, is it? Is it a parasite?

Based on prior findings of adult tortoise beetles on this particular morning glory vine, I'm assuming this larva is a Mottled Tortoise Beetle.

Appendage
It is some kind of appendage that they use to spread the fecal matter over the umbrella, maybe an extension of the intestine itself. I caught it in video once, but your photo shows what it looks like a lot better. What do you want? If you build an umbrella with your own feces you need some way to put them there.

I've seen it referred to as a "fecifork."

 
Coat rack
I think that the fecifork is the "coat rack" that carries the shed skins from previous molts plus all that mess. This could be called a feci-hose.
