Clubtail
This is a Clubtail, Jim. The strongly separated eyes are diagnostic for that family (and the Petaltails, but they don't have all those yellow markings). Unfortunately, I don't have much experience with Clubtails, so you'll have to wait for someone else to nail down the species.
