This ambitious and extremely detailed monograph was published over 24 years in seven "Numbers" (or "fascicles"). The bulk of the work, Numbers 1-6 (consisting of pp. 1—656), were published between 1980 and 1987, with the final Number 7 (pp. 657—716) appearing in 2004. On pg. 708 of that last part, the authors expressed their apologies for not being able to complete the intended treatment due to unforeseen circumstances, and the hope that other authors might continue work on the portion of the family not completed.
