This is the Andromeda Lace Bug, a lace bug import from Japan believed to have been introduced into the US with one of its host plants, the ornamental Pieris japonica or Andromeda plant.

is very useful on the identification of lace bugs. Luckily, this is a very distinctive lace bug and I was able to identify it without the host plant information.
