I vote for C. alternifolia! See 2nd image.
As for the gall ID, I'm assuming this is an early stage of the midge blister gall... it might actually be a fungus/disease, though.

Host plant...
...is some kind of dogwood (Cornus), roundish leaves suggest C. rugosa.
