After much deliberation, I think this has got to be a free-living caddisfly (Rhyacophilidae). It has the deeply incised segments, the U on the collar, and the large, hooks on the posterior end. My confusion arose from finding this individual (and others) in a case. I believe I was finding them in recently constructed cases just as they were preparing to pupate.
