Unknown moth - Peridea angulosa

Unknown moth - Peridea angulosa
Hamilton County, Ohio, USA
August 31, 2013
We have a lot of forest tent caterpillars, but this does not look that much like a forest tent moth. The circular structure on the head is unusual, and I don't see any obvious antennae. Is it possible that this individual has been injured?
