I don't think O. quadrimaculatus occurs in eastern states. This looks like O. ovalis.

 
Species key
Take a look at the key in "A revision of the nearctic Optioservus (Coleoptera: Elmidae), with descriptions of new species." To be certain, check genitalia if male, but I'm pretty sure it is O. ovalis.

body shape suggests otherwise; hope someone figures it out
